The South and West Hertfordshire HCP covers the following areas:
Current partners are:
- NHS Hertfordshire and West Essex ICB
- West Hertfordshire Teaching Hospitals NHS Trust
- Hertfordshire Community NHS Trust
- Central London Community Healthcare NHS Trust
- Hertfordshire Partnership NHS Foundation Trust
- Hertfordshire County Council
- GP practices
To support priorities in our area, the HCP has been delivering positive change in several areas.
One of these areas is the virtual hospital. The hospital was started at the beginning of the COVID-19 pandemic to care for people in their own home. It was awarded the BMJ Respiratory Team of the Year in recognition of the innovation.
The hospital has since evolved into a highly successful integrated service, run by the HCP. The care it provides now extends to people with chronic obstructive pulmonary disease (COPD) and heart failure. Over 130 people have been treated so far, with good feedback from patients and staff.
We are also working on the development of an extended primary care diabetes service. This will help local people get support to manage their condition at an earlier stage which will help prevent more serious illness.
Other priority areas include:
- advanced care planning
- children and young people’s services.
The HCP is also passionate about making sure those who live in south and west Hertfordshire have their say in what the partnership does and what changes need to be made. We are working with Healthwatch Hertfordshire to put in place some productive ways to involve local people and design changes with them.
Over the longer term, our partnership wants to reduce health inequality, prevent people from becoming ill, and ultimately improve the health and wellbeing of people who live in this area.
